The Doctor crashes into Equestria after a severe malfunction with the Tardis. After he is rendered unconscious, he wakes up to find he's become a pony, and as soon as he steps outside, he is greeted by Ponyville's resident Super Duper Party Pony, Pinkie Pie. After a storm of questions, and a hunch, Pinkie informs him of the issue of doppelgangers of the town residents, mostly notable in large crowds. Concerned, he immediately enters the town to solve the problem and he already thinks he knows who is behind it.

Why is the Doctor a Unicorn in the picture? Isn't he usually an earth pony?
7556743 A valid question, Starlight. This is my own personal version of the Doctor, a combination of my personality and that of the 11th Doctor. I haven't seen a lot of unicorn incarnations of the time pony so I thought it would be a fun prospect of having the Doctor be a unicorn, and have no idea how to use magic, whatsoever. If you are concerned of the Doctor's magical abilities becoming a deus ex machina, it won't. All will be revealed.
