An orchi is a slang term for an orchidectomy, better known as a castration. Obviously such a thing would be terrible for cis guys, but an inexpensive and effective way for trans women to balance their hormones. I've heard it called the "poor man's Sex Reassignment Surgery".

Try and think of it from her perspective. Street was a pony that kept getting so exhausted she couldn't perform her duties. She did not take medical leave and instead continued to put the ponies working her in danger by keeping coming to work after forgetting her pills.
Now, Street did try to minimize the damage and get leave, but by the time she figured things out she was already on the chopping block. From Sunny's perspective, she's trying to save Swift from the danger that is Street.
Street could potentially defend herself, but... Well... She doesn't have very good self esteem. It's hard for her to imagine any of this stuff not being solely her fault instead of her supervisors.
