One of a few semi-plausible theories. Celestia's theory, mass mind control, or some sort of transmogrification spell all fit as well, or magic they've never heard of before.
Besides, they've only encountered three alternate universes, that of the Breezies, that of the Humans and that of Discord. They have no idea if there are an infinite number of universes or just a discrete few.
Also this doesn't change the fact that whatever the mechanism of what happened to them, they have no other idea of how to stop the Mane 6 from further degrading without giving their condition some treatment... and if you're right Butterscotch definitely wouldn't want to come back to a dead body.
6068196 Well if that is their primary concern maybe they should be investigating what has been done to poor Fluttershy's mind that has made her so weak. Because Shy has never been weak. She has proven many times that her core is steel, not iron, but steel. We have seen her transformed more than once and she has never just fallen apart because of it. She isn't being bullied or ostracized, like she has been before as a foal and still survived. Her friends and family haven't turned on her. She hasn't suffered any of the outside stress or pressure that drives people to such extremes here in the real world... gotta be really redundant here before someone flips their shit and starts calling me prejudiced.

Why, I reckon that pointing out that possibility to her and mentioning how she is just taking care of poor sweet Butterscotch's body for him until they can fix things would be a perfect way to bring out the old 'any-thing-for-others' steel resolve we all know and love.
So? We have no confirmation of any other realities and we still have these theories. The concrete knowledge of three gives my theory far more credibility than it would have here and makes it more obvious, not less. Especially since one of the three is a related world containing other versions of themselves. Though I wasn't actually factoring in EG until you brought it up.

I will say that Fluttershy is an extremely strong character. There is no question about that, but most of her moments of strength have come from standing up for others, not standing up for herself. She's also arguably been given the largest curve-ball as she was used to a quite "feminine" body before.
Also... Just, generally I can say from experience that dysphoria hits different people in different ways. Some times the depression is mild and sometimes it's really bad. Flutters got the short end of the stick as this is something that can throw her into a spiral. She's depressed so she's too scared to get out of bed for a bit, which makes her feel powerless which makes her feel more scared...
There are all different kinds of fear.
However, she's not the one with the worst dysphoria, she just wears it on her sleeve. Someone else is in serious trouble.
Just popping in to express my continued interest. I can see Dash and Twilight as being bi fairly easily. The other four seem hetero to me, IMO, with the possible exception of Fluttershy. Still, the reveal seemed rushed.
At any rate, I see some grammar problems and missing/repeated words here and there. Proofreading is good.

Ugh... Being trans is complicated. When you tell people you don't like your body a certain way, they start telling you you're crazy or have mental issues or what not. Hear that enough times and you'll start to believe it. And since the standard treatments for mental illness have proven rather ineffective in dealing with gender dysphoria... You can see how things can go South pretty quickly.

It's a sort of catch-22 situation to be transsexual, as far as dealing with other people. They'll say you're crazy, and you very well might be, for taking expensive, heavily restricted, and dangerous chemicals, that probably will shorten your lifespan, just to feel more like a woman (not quite as life shortening the other way around, IIRC). But then, if you force yourself to ignore your weird body, and say no to the unsatisfying chemical solution, they'll say you're just a poser and it's all just pride and denial. And who are you to correct them?
Personally I think anyone should be able to say "yeah I want to be a <what I'm not>" and nobody has any right to say it's not real or genuine, or demand that you "prove" it somehow. Still a sucky situation, but at least people aren't treating you like a mental patient.
Ugh, and don't even get me started on the femynysm SJW thing, where people are all "you only want to be a girl because it's popular now," or "because you think girls are better than boys," or "because the evil feminists infected you with man guilt," or something like that. It's sad, because most trans-women are gay or at least bi/gay, so you could say they have a pretty healthy respect for men, outside of their own personal situation!
At least it's not flipped around like in the Victorian era where they were all "You just want to be a man because men are superior and you are just a simple animal wishfully imitating your betters, like a parrot." To be honest, this seems like a really realistic way for applejack to deal with this. Stress can be a real killer.
I don't think I entirely understand why dysphoria is affecting the mane six so violently. Ill admit that I'm no expert in this world,but wouldnt their knowledge that the cause of their gender change is magical make it much easier to mentally process so they would find it weird and confusing, but not be affected by the main theme that makes it so difficult to normally handle, uncertainty.
Also I can't help but feel that they've fallen into a parralel universe and when they do switch back the stallions will be royally screwed, but I know that isn't where you are taking this as you've put too many time skips in to follow that plot line. So I understand what the story is about, but a part of me is concerned that they are all ignoring what makes their case different in favour of showing solidarity with the trains group. Or something,I don't know. I keep getting thrown by how they propose ideas rather the whole gender part.
Then again its probably just that I have little experience with the topic so I have no frame of reference to understand their plight. So interesting story,and something that I'm going to reading, but I'm becoming aware that I'm not your target audience.

Something you should note about dysphoria is that it is, on a certain level, chemical. Brains are not designed to function on opposite gender sex hormones so they cause all sorts of misery when a person is off them.
And, I really hope that this story can have a little something for everyone. It does involve trans issues, but I hope to expand it into something more.
This story has me annoyed. The main 6 seem to be cavalierly planing to do irreparable damage to bodes they are only guests in. not to mention the reputations associated with those bodies. twilight has been to an alternate universe where her friends are a different species so why would she not think about being in a brain of the multi verse that has them as stallions. Meanwhile if luna and discord have their way when they switch back they will have made the people they love have massive body issues. If you do have them transition in this story and then have them switched back to their universe please don't have them transition back to being their normal selves but to a mares body that has been pumped full of male hormones. otherwise the implication is that only the men were the ones that were responsible and realized the situation and then were royally fucked over by their female counterparts.
