Interesting, if not a bit frustrating. I will never understand how the sonic rainboom wasn't a more well known incident. Like, was Pinkie really the only pony in the area who saw the giant rainbow explosion? No one in Equestria wrote anything on it? Heck, I never understood how the colts who Rainbow Dash was with when she did it could deny it was real... Maybe it was fake? How else could such a thing be ignored.
Anyway, I hope Pinkie keeps seeing Surprise after taking the medication, but that's only because I want Surprise to be real. Then again, if she is real, she's doing a pretty bad job of being a guardian angel. Pinkie is currently suffering because Surprise keeps telling her to proclaim her existence to others, which she should have known would cause problems for her ward. A real GA probably would have told her not to tell others about her.
Hope you get to the Mane6 sometime soon, it will be interesting to see how they react to Pinkie's first friend. Maybe Twilight can prove her real with magic or something.

Holy hot damn! I used to ship Surprise and Pinkie Pie... (don't ask) so this was a huge blast form the past! I'm glad to see people even know who surprise is! Only if maybe sometime in the near future use Firefly in a different story... Now that's something I would read... A like and favorite from me, keep writing!
