Canterlot General Hospital - Patient Transfer Form

Name: Sparkle, Twilight
Age: 22
Sex: Female
Race: Unicorn

Date of Admission: 02/01 - 10:00 AM

Doctor in charge of the patient: Dr. Tender Care, M.D.

Primary Diagnosis:
Patient Twilight Sparkle presented severe physical trauma at the time of her arrival, ranging from external bruising and lacerations to fractures and several concussions. Vital levels were below safe ranges, and had to be stabilised in the Emergency Room.

Principal Diagnosis:
As a result of her accident, Twilight Sparkle entered a coma state from which she could not be awaken. Nervous system had suffered a thaumaturgic feedback, causing severe damage to the brain. She is being admitted for Long Term Care under the supervision of Doctor Hayes.

Overall Goal for Patient/Prognosis:
Patient is of young age and has had no prior injuries, so all physical trauma should heal properly. Expect to be back in shape in six to seven weeks. Extent of neurological damage still unknown, but available information does not seem favourable. Final prognosis shall wait until we have further information.

Additional Observations:
Doctor Hayes, I know you’ve been personally asked by Princess Celestia to oversee Ms. Sparkle’s case. You’ll get my full discharge report later today, but I just wanted to let you know that there’s something odd about her. We ran the routine checks for patients who’ve fallen into a coma back when she was admitted and her Glasgow results are worrying, but it’s incongruous with her thaumaturgic readings. I know we have to wait for the result of the CAT scan, but what we have now says she should be able to at least react in some way. She hasn’t suffered brain death, for the Princess’s sake! We should be able to get something from her.
