Based on the size of baby Flurry Heart's wings, I'd find it very understandable (perhaps even likely) for Cadence to believe she was pregnant with triplets instead of just one baby. Hell, she probably felt like she was pushing out three babies at once with those freakishly massive/ prematurely developed feather-flappers attached.
