lets see, that's a tough one honestly.... once in high school I had mixed up some of my mental scheduling( our school had this scheduling bit with 4 different half hour periods for lunch) and ended up walking into one of my classes on the wrong day. Another a couple years ago I went white water rafting with my dad and sisters, it was a thing where they record video of the rafts at various points, and near one of these points I fell out of the raft and when I was being pulled back in my swimming trunks fell bellow my ass and that got recorded on camera. I think my sister still has that footage somewhere, not online though.
