I'm glad it wasn't TOO punishing. I had some preliminary comments that the ending was too improper because she didn't really react to Spike attacking her. Yes, she called his penis tiny but he did a lot of stuff to her.
And I don't like serious or permanant damage so my fetishy stories avoid that.
I liked it. It actually seems very fitting that their first time he is too small, and a (admittedly somewhat natural) reaction from Rarity causing him to go a little crazy and 'dragon up' (yeah I couldn't help myself) into something too much for her to handle. However Generosity being what and who she is, is clearly the best one to get him to calm back down and get under control.
Can for the life of me see why this got the downvotes that it did. Probably those who dislike clop (though this has a rather good story and lesson to go with it) or those that just disagree with the idea that Spike might harm Rarity.

I see that I may have been a bit too worried based on the reaction of my collaborator. I didn't see anything wrong with Spike getting big and lustfully crazed (it was his suggestion) because it seems canon, but rather than greed he is pushed by humiliation and unfulfilled horniness. It's why he spanks her rear and teats, forces oral and even makes her kiss his ass. She's facing humiliation. And gets that dash of ironic punishment, she wanted more than Spike's initial meat and she got it.

Well when you think about it LUST can simply be a differing form of GREED. So again, having been pushed that far and then unable to finish, IMO at least, would be a valid reason for his Greed to kick in. He wants more and isn't getting it.
As for her not reacting to his assault of her, it is still fitting in with her character. Again IMO. She really does love Spike, and this time his change was a direct result of her actions. Having seen the monster he can become before she can understand that he isn't entirely in control of his actions. Thusly, and especially since she was the reason he changed, she forgives him because he is willing to forgive her for her own actions.
Edit: I will say this much. It would have been most amusing had he not entirely changed back but rather Rarity finding she like a certain amount of abuse and domination. Though that could just be my, admittedly very twisted, point of view.

In a way that happened. He DOES end up larger, sized right for a pony, able to be a decent lover for her. Her sexual tastes can be up to interpretation so she may ask for a spanking now and then.
I was grokking you until the rapey sequence. That was highly uncomfortable. I appreciate that it was then treated as something about which Spike was ashamed, but the justifications were way too close to how actual rapists (and apologists) justify ignoring consent as well as perpetuating the rape culture. So I certainly don't see it as glorifying rape, and I'm sure you didn't intend it as such, but it certainly might warrant a trigger warning for folks who might have been victims of rape.
Besides that, I found Rarity's reaction to Spike's smaller than average phallus a bit uncharacteristic. Given her general nature, tact, and the fact that most women don't get orgasms from penetration, anyway (as much as people like to think that's the only proper way), and Spike definitely is shown to have the equipment to please in the usual way, I can't see her reacting like that. I would think she would be more... generous.

This is about the reaction I had anticipated based on what my collaborator noted. Sorry about this. It was about the worst relationship writing fumble I have ever done and I hold more shame over this than any other story.
The reaction was mostly a suggestion based on other popular depictionsof Rarity and because I needed a moment to show the size difference for contrast. Again, not my finest moment. About the only things I like in here are the fetishy bits, because they are mine. Not the rape part but the rest.

Well, writing any kind of sub/dom sexual scene is going to be a little tricky like that, naturally. I think it wouldn't have been quite so jarring if Rarity's dissatisfaction had been more subtly implied, perhaps over protests that she really was enjoying herself quite well, thank you, and if Spike had been described as less calculative and vengeful and more straight-up losing himself... kinda like you portrayed dog!Spike, actually, in that he just stops thinking for a bit and goes overboard. There would still be the risk of triggering someone, granted, but would make it seem a little less creepy that they just brushed it off and kept going at the end, maybe.
